  • What is your and your dependents status and location (including address)?
  • Did you and/or your dependents suffer any injuries/casualties?
  • Was there any damage to your property?
  • Do you have any additional information you want to pass, e.g., instructions for your supervisors, additional contact numbers/information you had not provided previously, etc.?
