Pollution Removal Funding Authorizations (PRFAs)

                

  1. Download and complete the appropriate form below.
  2. Attach the Statement of Work (see sample below).
  3. Sign the PRFA.
  4. Fax a copy to the NPFC Case Officer.
  5. Later submit the completed document with the Incident Report and Transmittal Form to the NPFC Case Officer.            

 

 Instructions Chapter 8 of the Resource Documentation TOPs provides detailed instructions for using and completing the PRFA forms.

Federal Agency PRFA

 Download and complete this form to establish a PRFA with a federal agency.
Non-Federal Agency PRFA
Rev. 04/23		 Download and complete this form to establish a PRFA with a state or local government.
Amendment to PRFA Download and complete this form to amend an existing PRFA with either a federal or non-federal government agency.
