Downloading Files

Most of the cost documentation forms are available in both an Adobe Acrobat (PDF) and a Microsoft Word or Microsoft Excel format.

File Type When to Download Symbol
Microsoft Word or Excel File
  • You have Microsoft Office installed on your computer
  • You want to complete the form electronically to use the automated calculations embedded in the files

(If a warning message appears when you open one of the Microsoft Word or Excel files, select Enable Macros--but only for files from this site)

  • You do not have Microsoft Office
  • You can't download large files,
  • You prefer to print and complete the form by hand
