The U.S. Coast Guard has adopted some of FEMA's published ICS forms to conform to the national standard. Other forms (ICS ###-CG) have been adapted from Wildlife NIMS to fit  more all-risk, all-hazard responses by the CG, and are not recognized by NIMS. The USCG ICS Forms are one of several tools that are available to support personnel managing a response to an incident and in completing an Incident Action Plan (IAP). 

The USCG no longer recognizes the "-OS" forms which were our original "oil spill" adaptations and have put "-CG" in the title of adapted or created forms. Forms without '-CG' have not been adapted at all and can be found in the NWCG forms section below. All wildfire NIMS forms may be used instead of the Coast Guard versions.

USCG forms must be printed locally and cannot be ordered. Wildfire NIMS form can be ordered from the NIFC.

IAP COVER SHEETThe CG Incident Action Plan (IAP) Cover Sheet provides a standardized way to organize and present all components of IAP to incident personnel.
ORGANIZATION ASSIGNMENT LISTThe Organization Assignment List (ICS 203-CG) provides ICS personnel with information on the staffing structure for the incident including the units that are activated and the names of personnel assigned to each position/unit for the planned IAPcycle. This form is used to complete the Incident Organization Chart (ICS 207-CG) which may be included in the Incident Action Plan and should be posted as part of the Situation Unit Display. Staffing of ICS positions is event-specific and not all positions are required be filled. Some blocks may also contain more than one name. The size of the ICS organization is dependent on the size and complexity of the incident which can be expanded or contracted as necessary.ICS-203-CG11/1/2024
ASSIGNMENT LIST ATTACHMENTThe Assignment List Attachment form (ICS 204A-CG) shows more specific instructions for specific resources if needed. This form may be used to support ICS-204-CGs for the Operations Section or the Intelligence/Investigations Section. A separate sheet is used for each Division or Group. The identification letter of the Division is entered in the form title. Also enter the number (roman numeral) assigned to the Branch. The need for an ICS 204A-CG is determined by the Planning and Operations or Intelligence/Investigations Section Chiefs during the development of the Operational Planning Worksheet (ICS 215-CG) development.ICS-204A-CG11/1/2024
INCIDENT RADIO COMMUNICATIONS PLANThe Incident Radio Communications Plan (ICS 205-CG) provides information on all radio frequency of trunked radio system talkgroup assignments for each operational period. The plan is a summary of information obtained from the Communications Resource Availability Worksheet (ICS 217-CG). Information from the Radio Communications Plan on frequency assignments is normally noted on the appropriate Assignment List (ICS 204-CG).ICS-205-CG11/1/2024
INCIDENT BRIEFINGThe Incident Briefing form (ICS 201-CG) provides the Unified Command (and the Command and General Staffs assuming command of the incident) with basic information regarding the response situation and the resources allocated to the incident.
