2015 W-2 & 1095 Forms Update

2015 W-2 and 1095 forms are now available via Direct Access self service. How to View and Print Your W-2 Form (PDF User Guide)

  • W-2s Missing State Code (Resolved): If your mailed W2 did not contain the state code in Block 15, you may log in to Direct Access Self-Service to print a W-2 containing the State code.
  • Documentation for Alaska Permanent Fund Dividend: PPC has submitted the file of all Coast Guard members claiming Alaska residency to the State of Alaska for the 2015 Alaska Permanent Fund Dividend. Coast Guard members claiming the PFD are not required to provide a copy of their 2014 December LES as the State of Alaska already has your proof of residency.

The Coast Guard Pay and Personnel Center (PPC) is in the process of printing and mailing calendar year 2015 IRS tax forms 1099R and W-2. IRS Forms 1099R and W-2 will be mailed by January 31, 2016.

In accordance with the Affordable Care Act, PPC will also be mailing Form 1095, Health Care Coverage. On 28 December 2015, the IRS extended the deadline for agencies to mail IRS Form 1095 (https://www.irs.gov/pub/irs-drop/n-16-04.pdf) to 31 March 2016. IRS Form 1095 will be mailed out in accordance with IRS regulations prior to this deadline. It is important to note that IRS Form 1095 is not required to complete and file your 2015 individual tax return. IRS From 1095 will also be available in Direct Access after mailings are completed.

Mailing of Wage and Tax Statements

1. IRS Form W-2 for 2015 will be mailed no later than 31 January 2016. W-2s are mailed to the member’s mailing address in Direct Access.

2. In addition to W-2 forms, PPC will also be mailing IRS 1095-B, Health Coverage forms. The IRS 1095 series forms include information already provided to the Internal Revenue Service. These forms contain information members need to complete their individual federal income tax return. See our "Your taxes and the Affordable Care Act" page for more information about these forms.

3.  In order for PPC to furnish the IRS Forms in a timely fashion, all members must ensure their correct and current mailing address is entered in Direct Access no later than 7 January 2016. To view and update your mailing address using Direct Access Self-Service (https://portal.direct-access.us/), go to the Main Menu and select Self Service, Employee, Tasks, Home and Mailing Address. Refer to ref (a), section 8.B.6, for address format examples for single members living in barracks or shipboard berthing.  Members must ensure forwarding of mail through the U.S. Postal Service if there has been a recent change in mailing address.

Requesting a duplicate Wage and Tax Statement

For 2015 and later W-2 and 1095 Forms, active duty and Reserve members will access forms via Direct Access Self Service. Do not submit a request for a replacement 2015 or later tax form, they will be avaible via Direct Access.

For 2014 and prior year W-2 forms, active duty and reserve members shall complete form CG-2016, W-2 Request Form (http://www.uscg.mil/ppc/forms/). Attach the completed form to a PPC Customer Care trouble ticket at http://cglink.uscg.mil/PPCticket or via e-mail to ppc-dg-customercare@uscg.mil. If submitting via CG Workstation, a digital signature is preferred.

For 2015 and prior year W-2 forms, separated or retired members shall complete form CG-2016, W-2 Request Form (http://www.uscg.mil/ppc/forms/). Attach the completed form to an e-mail to ppc-dg-customercare@uscg.mil.

Replacement W-2 forms will not be available until 15 February. This is allow time for the original mailed form to arrive at its detestation or to be returned as undeliverable.

CG-2016 forms received via fax are no longer accepted and will not be processed.
