1. REQUEST LETTER:

The mariner training provider should submit the request (by email, fax or letter) to the appropriate Regional Examination Center 21 days prior to the course offering and contain the following:

  1. Name(s) of Coast Guard approved course(s) to be offered at the proposed site.
  2. Course(s) dates.
  3. Number of students enrolled, not to exceed 24.
  4. Name of approved instructor giving the course.
  5. List of training equipment to be used the proposed site.

2. SITE PLAN:

A scaled site plan must be provided for each training facility location request. If classes are to be held in a local alternate training facility, the site plan, when necessary, should be accompanied by marketing brochures or other reliable information, such as signed copies of contracts (see note) to support the information set forth in the scaled site plan. The site plan should include:

  1. Arrangement of student desks.
  2. Instructors area.
  3. Training aid placement.
  4. Aisles, doors and fire exits marked.
  5. Digital pictures of the classroom(s) to be used with the desk, chairs and training aid/equipment in place.

NOTE: If a mariner training provider desires to make a alternate site approval request for multiple future dates at the same site location, then it is suggested a copy of a lease agreement be obtained from the alternate site location (hotel, conference room, etc.) stating that a classroom space equivalent to or larger than the Coast Guard approved classroom space will be available for use by the mariner training provider for the requested dates and duration. If a lease agreement is not provided that covers future course dates, the alternate site approval will expire on the date following the end of the last course offered.
