Common Errors On Application
 

 

MEDICAL EXAMS, TESTS, AND DEMONSTRATIONS OF PHYSICAL ABILITY. All exams, tests, and demonstrations must be performed, witnessed or reviewed by a physician, physician assistant, or nurse practitioner licensed by a state in the U.S. , a U.S. possesion, or a U.S. territiory. Exams, tests, and demonstrations performed, witnessed, or reviewed by holders of foreign medical licenses and chiropractors or naturopathic doctors are not accepted under current regulations. All applicants who require a general medical exam must be physically examined. Examinations based solely on documentary review and/or patient history review are unacceptable. * See 46 CFR 10.205(d), 10.207(e), 10.209(d), 12.02-27(d), 12.05-5, 12.15-5, & 13.125.

 

Top 10 Common Errors

There are errors that can delay your application for credentials or cause the application to be denied.

 

Common Mistakes Mariners Make When Submitting the CG-719K:

  1. No response to medication section (Section III)
  2. YES or NO response not provided for each condition listed on page 5 (Section IV)
  3. Incomplete details (date of diagnosis/resolution) of conditions identified (Section IV)
  4. Missing uncorrected vision (Section V)
  5. Uncorrected vision tested with corrective lenses (Section V)
  6. Missing field of vision (Section V)
  7. Inappropriate color vision testing method, number of errors omitted or determination not indicated (Section V)
  8. Hearing not marked as normal or impaired (Section VI)
  9. Missing pulse rate/rhythm (Section VII)
  10. Provider failed to sign/date the form and provide his/her license number (Section IX)

Physical Exam - If the Merchant Marine Personnel Physical Examination/Certification Report is not complete, it will be returned for correction. Particular attention is paid to the "competent", "not competent", and "needs further review" boxes, which are frequently blank. Often the type of color vision exam given in Section IV is not indicated or mariners who wear glasses and/or contacts submit exams without their uncorrected vision listed in Section III.

 

Medical Condition - Additional medical information is required whenever a significant medical condition is identified on the Merchant Marine Personnel Physical Examination Report.
