HSWL RP Kodiak
USCG C-130 MEDICAL FLIGHT

MILITARY ID & TRAVEL ORDERS ARE REQUIRED*
MEDICAL RECORD AND ALL X-RAYS/MRI’S ARE REQUIREDKodiak to Elmendorf Air Force Base:
- Showtime: 0545 in the PAX terminal Hanger 1. Do not be late! All late personnel will be bumped to space "A" and may not make it on to the flight.
Elmendorf Air Force Base to Kodiak:
- Showtime: 1645 at the EAFB PAX terminal. Do not be late! If you are going to be late, contact the Off-Island care representative at 907-654-0474.
Upon arrival to the EAFB PAX terminal, there will be a shuttle bus for medical patients only. Designated drop off points are at the BX/PX and then at the EAFB Hospital. Return pick-up for both the BX/PX and Hospital is 1630. Do not be late!
All the follow-up appointments should be scheduled for a future Med flight. The Med flight schedule is attached. Notify the Off-Island care office at 907-487-5757 ext 119 once back in Kodiak so travel arrangements can be made.
Upon returning to Kodiak, schedule a follow-up appt with your referring Physician for the next available opening.
If authorized a rental car, a list of rental locations is attached. If no reservations are available, or you are not able to drive, a taxi is authorized.
If authorized Lodging, you must utilize the NORTH STAR INN at 907-552-2454. If no rooms are available, you must obtain a Non-available statement from the front desk prior to using commercial lodging.
If delayed overnight for medical reasons, you must have written documentation from your medical provider and fax to off-Island care at 907-487-5833. It is your responsibility to contact SATO for return airline tickets at 1-800-919-7286.
***(If you must use SATO, make sure they give you a "Locator number". Once you receive your amended orders, you must write this number on them and fax them back to SATO as confirmation. Their fax # is 866-914-7386.)***
Flights are not always guaranteed. Be prepared to spend the night.

USCG MEDICAL FLIGHT COMMERCIAL
It is your responsibility to contact SATO for airline tickets @ 1-800-753-7286. Flights must be scheduled according to the dates allowed for travel which are stated on your medical orders. Once you have your reservations, make sure SATO gives you a "Locator Number". This number must be given to an Off Island Care representative at 907-487-5757 x119 prior to picking up your orders.
All follow-up appointments should be scheduled for a future Med Flight. The Med Flight schedule is attached. Once you have returned to Kodiak, notify the Off Island Care office during normal business hours so that future travel arrangements can be made.

If you are delayed for medical reasons, you must have written documentation from your medical provider and Tricare authorization before adjustments can be made to your travel. Please fax the doctor’s recommendation to 907-487-5833.
If any problems or unexpected circumstances arise, contact the Off Island Care Rep at 907-487-5757 x119 during normal business hours. If calling after hours, contact the Rockmore-King Clinic watch captain at 907-487-5757, option 0. Any changes to your orders must be accompanied by written documentation.
