Medals and Awards Requests

There are three different avenues for veterans, retirees, family members to get copies of their personal or family members' awards, depending on need:

For requests for awards verification or for copies of awards or records for personnel who were discharged/deceased/retired before 1/1/1898, the requestor should submit an SF-180 to:

National Archives & Records Administration
Old Military and Civil Records (NWCTB-Military)
Textual Services Division
700 Pennsylvania Ave., N.W.
Washington, DC 20408-0001

For requests for awards verification or for copies of awards or records for personnel who were discharged/deceased/retired on or after 1/1/1898, the requestor should submit an SF-180 to:

National Personnel Records Center
(Military Personnel Records)
9700 Page Ave.
St. Louis, MO 63132-5100

For current active duty and reserve members, and requests for awards not previously authorized/received to a former service member, the individual should submit their request in writing with supporting documentation to:
