Description of the incident:
On 11 January 1992 a Bell 207 Jet Ranger helicopter with a total of five
people aboard, including Lieutenant Carl Johnson, crashed and sank near
San Pablo Bay while on a low-altitude survey of oil residue from a
three-day-old spill. They went up in the contract helicopter owned
by Air There of Gnoss Field in Marin County to investigate reports of a
residual oil sheen in Carquinez Strait, a narrow body of water that
separates San Pablo Bay from Suisun Bay, 20 miles from San Francisco.
The helicopter crashed after hitting a power line while passing over the Carquinez Bridge, killing all aboard.

Description of the incident:
Lieutenant Commander William B. Turek, a marine inspector assigned to MSO
Hampton Roads, gave his life in the line of duty on 3 March 1993 while
inspecting the M/V Cape Diamond. During a test of the ship's
carbon dioxide extinguishing system a release of a significant amount of CO2
imperiled the crew stationed in the engine room. Lieutenant Commander Turek
gave his life in attempting to warn those men of the CO2 release. The Coast
Guard posthumously awarded Lieutenant Commander Turek the Coast Guard Medal.

Description of the incident:
On 13 July 1994 Senior Chief James A. Pavini and Chief Charles R. Blome, Jr., Marine Inspectors from MSU Galveston, were two of four passengers aboard a contract helicopter on their way to inspect a merchant vessel underway at sea off Galveston, Texas. The helicopter, an Aerospatiale AS-350B1, leased by Sea-Link Helicopters, crashed into the Gulf, killing all four passengers. The pilot was the only survivor.
