Periodic Health Assessment (PHA)

Required PHA
All Coast Guard personnel will receive a PHA annually during the member’s birth month. If the member will not be available during their birth month, the PHA may be performed up to 2 months prior to their birth month.  For recording keeping purposes (in MRRS), the entry date for the PHA shall be the date the patient is seen by the HS for the PHA appointment. When feasible, the PHA will be accomplished in a single visit.  The PHA will consist of the following components:

• Fleet HRA

• Clinical Preventive Services (CPS)

• IMR Review and Update

• OMSEP Review and Update

• Problem Based Examination
