| The U.S. Coast Guard is investigating the loss of hard-copy internal Quality Assurance reports from our Office of Work-Life that may contain personal information on some of our current and former members and civilian staff. We became aware of the missing reports following a recent office move from the Headquarters Building. The reports document Quality Assurance Management visits made between 1997 and 2006 by Coast Guard Headquarters staff responsible for the Family Advocacy and Special Needs Programs. They do not contain full case histories, but some may contain combinations of names, social security numbers, and a brief description of the type of special needs or family advocacy case. Although there is no evidence of misuse of the reports at this time, in an abundance of caution, we are taking the steps to notify all potentially affected individuals.
