- KEEP CONTACT INFORMATION CURRENT
– Direct Access This is how the SECTOR will contact you in the event of
a recall. It is extremely important that you keep your
contact information current at all times. SECJAX Active Duty
Command has NO tolerance for members with outdated or missing DA
contact information.

Business: Put you
uscg.mil address (or dhs.gov) here. Due to security purposes,
we are no longer allowed to enter 2 addresses separated by a
semicolon. If you have two email addresses separated by a
semi-colon here, please remove the non-uscg address. DA
automatically sends e-mails to this address. (TA, forgotten password,
etc)
Home: Put your primary email address here.
This is an address where you can be most likely reached during a recall
or in order to receive important, time sensitive information.
If you check your ‘civilian’ work (non-USCG) email more often than your
home email, put your ‘civilian’ work (non-USCG) email here.
Other: Not required. If you have an email
address for work (non-USCG) and one for home, put the one that you
check less often here. This may be an email address that you
check while you are at ‘home’!

Home and Mailing Address
Phone Numbers: Add
Home, Cell, Business phone numbers. The "Other Phone" field will be
used in the event of a Hurricane Recall. Please select 'Other Phone'
from the drop down and type the phone number of a relative or friend
that resides in a geographically different location than you. This is
in the event phone lines go down in your area. Type the phone number
(without dashes) and add the words "out of area contact" Example: “568
940 5678 out of area contact”
- Annual Screening Questionnaire (ASQ)
– Direct Access The required completion date for the ASQ is 31 October
of every year.
*** Reservists must complete the ASQ annually between 01
August and 31 October.***
SELRES members who fail to complete and submit an ASQ are considered
unsatisfactory participants. Commands shall ensure that members are in
compliance before scheduling and approving ADT Orders.

- Individual Medical Readiness –
Per COMDTINST M6150.3, assessing Individual Medical Readiness (IMR) is
a continuous process and must be monitored and reported on a regular
basis to provide service leaders and commanders the ability to ensure a
healthy and fit fighting force ready to deploy. IMR consists of six
elements: Individual Medical Equipment (IME), Immunizations, Readiness
Laboratory Studies, Dental Readiness, Deployment Limiting Conditions
(DLC) and Periodic Health Assessments (PHA).
It is the personal responsibility of each member of the Coast Guard
active duty and reserve community to maintain their medical readiness
levels at all times, including making and keeping their PHA and Dental
appointment and maintaining their current vaccination requirements.
Information regarding how to obtain your Periodic Health Assessment,
Dental Screening and vaccinations can be found on the Medical Section
of this website.
Failure to comply with medical readiness requirements will result in
supervisor notification, counseling and possible negative page 7,
placement into Individual Readiness Reserves (IRR) and/or discharge.
- WEIGH-INS - April
& October
If you can not weigh-in during April or October drill weekend, you will
need to make arrangements with SECJAX Active Duty Admin Personnel prior
to the end of the weigh-in month to be weighed in. Personnel
who fail to weigh in prior to the end of the applicable weigh in month
(April/October), shall be counseled and issued a negative Page 7.
- IDP - Complete Individual Development
Plan
This is mandatory for enlisted: E-6 and
below and officers: O4 and below.

Once complete, please ensure that your supervisor emails BMC Newberry
with your full name stating that your IDP and counseling has been
completed and who the counselor. BMC Newberry and MKCM Paine have TMT
access and can enter this on drill weekends.
- ENTER IDT DRILLS –
Direct Access
You will be expected to enter your own IDT drills into DA for the
remainder of the fiscal year.

- ADT – You will need to
submit a request in Direct Access for ADT orders to your supervisor at
least 45 days in advance.
IMPORTANT!!! This is NOT intuitive: Clicking the
“ROUTE” button makes the screen blink and gives you the impression that
your orders have been routed. WRONG!!! Nothing has
happened. You MUST hit “SAVE” after you click “ROUTE” to
actually get your orders routed.

- EVALUATIONS
Ensure that your Enlisted Evaluation Review is submitted by your
supervisor on time.
