There are 4 different request forms used for the purpose of access control. An access control application must be filled out completely for each individual over the age of 18 that you are expecting. Explanation of each form follows.

Each access control form is to be used for a specific type of access request.

It is also used when requesting access for a housing guest for a period of more than 10 consecutive days. Requests must first be approved by the Housing Office before it can be processed. Housing will either approve or disapprove the request and forward their decision to the Environmental Health and Safety Office. If approved, a housing access card will be issued for a period of no more than 6 months.
