Duty CG and Public Health service members.
CG Selected Reservist and CG Active
Reservist’s are authorized care when on
orders. These services
include, but are not limited to medical &
dental appointments, and referrals.
Check in your health record.
If you are hand carrying your health
record from your previous unit, be sure to
check your record in at the clinic’s front
Primary Care Management.
Be sure to contact a Tricare
This will ensure you’re eligible for
care in this region.
Medical/Dental enrollments for dependents
are also made through the Tricare website.
Health Record Review.
The clinic staff will review your
record to ensure that you are up to date on
physicals, immunizations, and dental exams.
Certified Medical Record Copies for
Medical to let them know you would like a
certified medical record copy one month in
will do a quality assurance (Q/A) check.
3. Once Q/A is
complete, the member requesting
the Certified Copy can come to
Medical to make a copy of the medical record
or request a copy be made by Medical.
4. Once the
medical record has been Q/A’d and copied, it
can take 3 to 4 weeks to verify and certify
