Monday/Wednesday/Thursday:               0700- 1600
Tuesday:                                                    0700-1800 / 0730-1130* 
*(Please note that the clinic is closed for appointments every other Tuesday beginning at 1130 for training)
Friday:                                                         0700-1130* 
*(Please note that the clinic is closed for appointments on Friday’s beginning at 1130 for Admin time)
Duty HS onboard M-F:                              1600-1800
The Clinic is closed daily from 1130-1300 for lunch.

Care Eligibility:
Base NOLA Medical Department offers care to Active Duty CG and Public Health service members. CG Selected Reservist and CG Active Reservist’s are authorized care when on orders.  These services include, but are not limited to medical & dental appointments, and referrals.
 
Reporting In:
1.  Check in your health record.  If you are hand carrying your health record from your previous unit, be sure to check your record in at the clinic’s front desk. 
2.  Primary Care Management.  Be sure to contact a Tricare Representative.  This will ensure you’re eligible for care in this region.  Medical/Dental enrollments for dependents are also made through the Tricare website.
3.  Health Record Review.  The clinic staff will review your record to ensure that you are up to date on physicals, immunizations, and dental exams.
 
 
Requesting Certified Medical Record Copies for Separation/Retirement:
1. Contact Medical to let them know you would like a certified medical record copy one month in advance.
2. Medical will do a quality assurance (Q/A) check.
3. Once Q/A is complete, the member requesting  the Certified Copy can come to Medical to make a copy of the medical record or request a copy be made by Medical.
4. Once the medical record has been Q/A’d and copied, it can take 3 to 4 weeks to verify and certify the copy.
5. Once a certified copy is made, the member will be contacted to pick it up.
