Base Los Angeles-Long Beach Personnel Support Department is one of several Defense
Enrollment Eligibility Reporting Systems (DEERS) and Real-time Automated
Personnel Identification Card Systems (RAPIDS) sites located in Southern
California. DEERS/RAPIDS services and information updating are
performed in accordance with current DOD and Coast Guard Instructions.
Our staff provides services to members of ALL armed services, retirees and
dependents.

Eligibility - Armed Forces
ID (AFID) Cards are issued to active duty, reserves, retirees, dependents
(10 years of age or older unless dependent not residing with sponsor), and
100% disabled veterans (DVA) from all services; DVA sponsors MUST have their
letter of entitlement issued by the Department of Veteran Affairs.
When applying for an AFID Card the following information applies:
Identification -
Previous AFID card which is surrendered when a new one is issued and TWO
other forms of government issued picture ID if the AFID Card is lost or
stolen. Dependents must be accompanied by the sponsor; or if not accompanied,
they must have a DD 1172 (Application for Uniformed Services
Identification Card DEERS Enrollment Form) signed by the sponsor and
witnessed by a DEERS Certifying Official. (EXCEPTIONS: form signed by
the sponsor with seal and signature from notary public is acceptable if not
certified by DEERS officials or Specific power of attorney for DEERS with
list of all enrolled/existing eligible dependents included).
Students - Age 21 and
23 (only if they are attending school FULL time), must provide a letter
from the school certifying the dependent is enrolled as a full-time
attendee with estimated graduation/semester end date and school
seal/stamp. (Transcript or number of units enrolled NOT
ACCEPTED). Members age 21 and over who do not attend full-time are
no longer eligible for an AFID Card and are not considered a dependent.
Marriages - All original
documents required as follows: Marriage certificate (marriage license
not accepted), birth certificates, proof of ID, and a social security card
are required for new marriages and none new born child US citizen dependents
(copies not acceptable). If a social security card has not yet
been issued to non-US citizen spouse/children, TIN/TAX ID number
can be substituted, or temporary ID with limited grace period will
apply in order to enter the dependent into into DEERS/RAPIDS for medical
purposes. However, the period of eligibility is limited and will
terminate unless the social security card is provided and DEERS/RAPIDS
is updated.
Newborn Children -
Original documents required as follows: Birth certificate and social security card
are required for a new born child. Newborns can be entered immediately following birth with
paperwork issued by birthing (Hospital certificate of live birth) center to cover medical until the above
documentation can be applied for and received; however, the period of
eligibility is limited and will terminate unless the birth certificate
and social security card is provided and DEERS/RAPIDS is updated.
All active and reserve military sponsors now have the ability to make address
changes and issue a digital authorization form for their enrolled dependents
when dealing with ID renewal and/or ID replacement due to lost, misplaced or
stolen ID without physically going to a DEERS office with their dependents. Sponsors
can log-in to RAPIDS Self Service portal https://www.dmdc.osd.mil/self_service using
their valid CAC card with access to CAC readers. (Forms with digital signature do not apply
to hard copies and will not be accepted).
