About the Program

Two Family Resource Specialists (FRS) are assigned to Work Life Boston to manage the Special Needs Program. They provide the following services. Please contact an FRS with any questions. We are here to help.

Contact an FRS to enroll. The applicable forms needed to enroll are listed and hyperlinked below.

DD 2792 Special Needs Family Medical Summary is to be completed by you/your spouse and medical provider when your dependent has a medical/psychological special need. DD Form 2792

DD 2792-1 Special Needs Education/Early Intervention Form is to be completed by you/your spouse and Special Education staff when your dependent child has an Individual Education Plan (IEP) (PPT for CT) at school. DD Form 2792-1
