PATIENT CENTERED WELLNESS HOME

A Patient Centered Wellness Home is a team of healthcare providers and support staff who deliver services for total healthcare, surrounding the patient with support to accomplish wellness goals. This healthcare model will include and coordinate the disciplines of medicine, dentistry, pharmacy, family support, work-life, and safety, forming the Patient Centered Wellness Home. Each patient is expected to actively participate in developing individualized health and wellness directives and goals with our team.

Hours of Operation: Monday- Friday, 0700 – 1130 & Monday, Tuesday, Thursday, Friday, 1230 – 1530
CLOSED Wednesday 1230-1530 for Training

ROUTINE CARE

The Clinic provides primary care services for Coast Guard active duty members by appointment. Appointments may be scheduled by calling the appointment desk at (718) 354-4414.

MILITARY OPEN ACCESS

This is held Mon-Fri from 0700-0800 for active duty with acute illness. Arrive at clinic at 0700 or call (preferred) front desk at 718-354-4414 to receive the acute care appointment time.

DENTAL CARE

United Concordia Dental network of providers is used for Active Duty personnel only. Please call United Concordia ADDP at (866) 984-2337 to acquire a dental access control number (ACN). This number is needed to schedule an appointment with a network provider. Marine Dental is most convenient at (718) 987-6543 for your ANNUAL exam (718) 987-6543. They are located at 255 Mason Ave, Staten Island, NY. Dental exams are required ANNUALLY for active duty/reserve members and should be completed within 30 days of your birthday.

PHYSICAL EXAMINATIONS

Services provided by appointment only. We provide a full range of required physical examinations to meet the operation readiness. Physical Exams can be scheduled by calling the centralized appointment desk at 718-354-4414.

PERIODIC HEALTH ASSESMENT (PHA)

Requirement varies according to type of work exposure from every 6 months to 1 year as designated by the physician. Contact your local OMSEP coordinator to be enrolled.

OMSEP

Requirement varies according to type of work exposure from every 6 months to 1 year as designated by the physician. Contact LTJG C. Hunt to be enrolled.

COMMAND AFLOAT

Required prior to assignment and annually thereafter.

OVERSEAS / SEA DUTY

Required within 6 months of reporting to an overseas or afloat unit that deploy for a period of 60 days or more.

PRE-COMMISSIONING / APPOINTMENT

Required within 12 months prior to appointment as an officer.

SEPARATION FROM ACTIVE DUTY

Required within 12 months for retirement and 6 months for involuntary separation or release from active duty into the Ready Reserves (selected drilling or IRR).

AFTER HOURS CARE

Our team maintains on-call services 24 hours a day to address urgent care needs. All active duty personnel are responsible for contacting their team Duty HS at (917) 679-4147 after hours prior to seeking urgent/non-life threatening care. EMERGENCY CARE – Go to the nearest Emergency Department or call 911. As soon as practical, notify the Duty HS and Command of the emergency room care and/or admittance to the hospital.

TRICARE SERVICE CENTER (TSC) The TSC will provide information and assistance related to health care benefits and is located across from CGPD. Walk-in services are offered M-W-F: 1200-1600 or T & TH: 0830-1230 or call 1-877-TRICARE. Signing up on www.mytricare.com is a quick and easy way to find providers, monitor referrals, authorizations and access to your Explanation of Benefits (EOB). Additional information regarding Tricare and provider searches can be found on the web at www.hnfs.com.

GRIEVANCES

All grievances or requests to change primary care managers will be addressed by the Health Services Administrator, HSC Walter Lipski at (718) 354-4341.

Should you need to contact your health care provider with any questions, please call the front desk at (718) 354-4414. The front desk staff will forward your information to the appropriate provider/HS, who will then return your call at their earliest availability.

Specialty Services

Specialty Services are available through Tricare Health Benefits. Discuss any health care provided outside the Sector New York Clinic with the Patient Affairs Office. A referral from you primary care manager is required to access the routine civilian care specialty services:

PHARMACY

Pharmacy services will be provided by local Tricare network retail pharmacies via written prescription from our medical officers. Coast Guard members are encouraged to utilize the Express-Scripts Mail Order Pharmacy program for any chronic medications. Sign up and order forms are available at our Patient Affairs Office and Express-Scripts can be reached at (877) 363-1303 and on the web at http://www.express-scripts.com/TRICARE/.

 

IMMUNIZATIONS

The immunization clinic hours are open during normal business hours. Patients must remain in the clinic for 20 minutes after receiving any immunizations to ensure there are no adverse reactions.

EYEGLASSES

Services such as eyeglass fitting and ordering are available Tuesday and Thursday afternoons from 1230-1500 on a walk-in bases.
