Physical Examinations

The CG Periodic Health Assessment Physical
This can only be completed within 60 days of your birth month, if this is not the case please familiarize yourself with process but schedule when your birth month arrives. The goal for this annual physical is to get all medical and dental readiness on the same schedule so you spend fewer visits at the clinic throughout the year unless you have an injury. This should be more convenient for everyone by reducing the number of trips made to the clinic throughout the year to update medical readiness.

1.      Visit the Health Risk Appraisal link (below step 5) and enter your entire OPFAC without dashes. Example:   Base NOLA is 083247710 and R083247710 for reservists.
2.     Call  504-253-4671 to set up the appointment. You will bring in the questionnaire with you from the HRA link below. The OMSEP periodic physical is included with this. It is your responsibility to ensure you bring the periodic OMSEP forms for your specific protocols in when you come. You can get these from the unit OMSEP coordinator.
3.      Your HRA will be reviewed with you to get family history and your health history and determine what tests you need based on age and sex. Your immunizations, dental and medical test readiness will be checked.
4.      If you are cleared and do not need any labs or to follow up with the doctor your PHA is complete. If you are high risk you will be required to make a follow up appointment to speak with the doctor. If labs are ordered for you, you may or may not be required to follow up based on the results of those tests ordered. If they are normal you will be complete, if they are not normal you will be asked to follow up.
5.      It takes 24-48 hours for your information to update in CG Central from the day you are seen. If that change does not occur please call  504-253-4671 to correct the issue.
This process is a little more in depth than the PHA listed above. Depending on the type of physical you require there will be specific tests related to it.
1.    Fill out the forms DD2808 and DD2807. The Clinic will send you the forms specific to Base NOLA once you have made contact requesting an appointment.
2.    Make an appt. This appt will take 40 minutes; it is for hearing, vision checks, lab tests and medical readiness.
3.    The Clinic will schedule the second appointment for you to see the medical officer when after the initial visit. This will cover you getting the lab results and an exam.
4.    If you receive any consults you will be required to follow up on them with the provider that performed your physical so that the final signature may be given on the physical. If you did not require any additional testing the remaining signatures will be acquired.
If you’re physical is for Separation, Retirement, Retention, or Commissioning it goes to the clinic administrator for approval. If it is for flight and aircrew it goes into the AERO program for approval. You will hear directly from the approving official.
If you are due for a periodic OMSEP physical then please schedule this with your PHA. That information is listed above.   If you need a baseline or enrollment into the OMSEP program please follow the same steps as the Commissioning Physical.
