Personnel requiring Official No-Fee Passports should complete and submit their applications at least 6-8 weeks prior to cutter deployment, to allow sufficient time for processing by the Department of State. The procedures for completing the passport application process are as follows:

A complete passport application package consists of a passport application, cover sheet, proof of citizenship and photos. Once the application and cover sheet have been completed, personnel must contact the PSD Customer Service Desk (CSD) at (510) 437-5371 to schedule an appointment with a Passport Acceptance Agent to take photos, complete the process and forward the application to COMDT (CG-DCO-I). Individual appointments are scheduled Mondays and Tuesdays from 0900-1100, and group appointments as mutual scheduling permits.

(1). Application. The passport application is located at:

Click "Passport Application Wizard"
Check "I have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaimers."
Click "Submit”
Click "Continue" under Apply for a Passport Book

Fill out all blocks as appropriate, or as specified below:
Occupation must read: "MILITARY" for enlisted, "MILITARY OFFICER" for officers, "CIVILIAN" for Coast Guard Civilian employees, or "DEPENDENT" for military dependent spouses and children.
Employer must read: "USCG" for all applicants.
Mailing Address must read:
Street address: COMMANDANT CG-DCO-I
Street address 2: 2100 2ND ST SW STOP 7471
Zip Code: 20593-7471
In care of: DHS CG
Permanent address may be unit or home address.
Phone number: after entering phone number you must click "ADD" for it to print on the form.

At the fee page, do not check any boxes, just click "NEXT"
Review all information and click "SUBMIT"
Click "CREATE FORM" to open Adobe PDF of completed form.
Print form, pages 5 and 6 only.

(2). Cover Sheet. Request for No-Fee Passport Form.

Complete only blocks 1-5 for MSST personnel.
Complete blocks 1-9 for all other military/civilian personnel.
Complete blocks 1-10 for military dependents.

Block 1: this will be the form number for your application, either a DS11 or DS- 82.
Block 2: enter Rank and Grade for military/civilian personnel, or "Dependent" for military dependents.
Blocks 3-5: self explanatory.
Block 6: must be the same as reflected in the itinerary on your application.
Block 7: be as specific as possible.
Blocks 8-9: must be the same as reflected in the itinerary on your application.
Block 10: self explanatory.

(3). Proof of Citizenship. Proof of citizenship must be one of the following:

Original birth certificate (if citizen born in the United States) or;
Original Report of Birth Abroad (if citizen born outside United States to United States citizen parents) or;
Original Naturalization Certificate (if naturalized) or;
United States Passport.

*Copies and abstracts will not be accepted as proof of citizenship by the State Department.

(4). Photos. Photos must be taken in a collared, civilian shirt that in no way affiliates with the military.

All passport applications are logged by PSD with the member’s name, unit, date sent and FedEx tracking number. Once the passport and member’s original proof of citizenship are received by PSD, the passport will be logged and the ship’s office will be notified for the passport to be picked up and signed for.
