SUBJ: DEPARTMENT OF HOMELAND SECURITY (DHS) FIFTH SECRETARYS AWARDS
CEREMONY REQUEST FOR NOMINATIONS
1.  Nominations are being accepted for the Fifth Secretary of DHS
Awards Ceremony.  The ceremony will be held in the National Capital
Region, January 2013 and honor individual and collective achievements
across DHS for the period of 1 October 2008 through 30 September
2012. Nominations are solicited for the following categories:
Exceptional Service Award (Gold Medal), Meritorious Service Award
(Silver Medal), Secretarys Award for Valor, Award for Exemplary
Service, Award for Excellence, Award for Outstanding Achievement in
Diversity Management, and Award for Volunteer Service.  For a
description of award categories and the Secretarys Nomination Form,

    b. Nomination forms must be typed, using Times New Roman 10 point
font, conform to space provided, be complete, and accurate and
appropriate.  Information submitted will be used in the awards
ceremony program and must be suitable for public consumption.
Include name, title, approving officials signature and date in block
(9).  The last line in block (9) must be left blank for Commandants
endorsement.
    c. Team nominations are limited to 10-15 employees, one form per
team and must include a separate one-page list of members by first
name, middle initial, last name, title, unit, component/office, and
point of contact for each team nomination submitted.
3. You are encouraged to recognize employees not previously honored
with Secretarial awards.  These awards are not intended to honor
retirees or contractors.  Nominations may not be submitted for
individuals/teams whose accomplishments are pending final action or
involve a pending investigation.  Nominations will be reviewed by the
Civilian Awards Board prior to Commandant approval, then sent to DHS
for final consideration.
4. All nominations must be received by (CG-1214), no later than 17
October 2012. Extensions will not be granted.  Incomplete submissions
or nominations that exceed three pages will not be accepted.
5. You are strongly encouraged to take full advantage of this
opportunity by nominating deserving employees for recognition in the
appropriate award categories above.
