In accordance with the National Defense Authorization Acts (NDAAs) 1998 and 2005, the military services are required to perform deployment health assessments for deploying members. Refs (A) and (B) implement policies and prescribe procedures for deployment health activities, including deployment health assessments. 2. The Coast Guard Business Intelligence (CGBI) system now includes Deployment Health Assessment metrics based on Coast Guard members participation in an expeditionary deployment. These metrics will display red, amber, or green depending upon whether a member is delinquent, pending, or up-to-date (respectively) with their Post-Deployment Health Assessment (PDHA), Form DD-2796 or Post-Deployment Health Reassessment (PDHRA), Form DD-2900. 3. Currently, 75 percent of Coast Guard members are delinquent with their PDHA, PDHRA, or both. Coast Guard members who have been on expeditionary deployments should review their personal page on CGBI to determine if they are delinquent. Completion of required assessments is mandatory. 4. Coast Guard commands should review the unit level reports in CGBI to ensure Coast Guard members who have been on expeditionary deployments complete the mandatory deployment health assessments. 5. Ref (A) describes the process for Coast Guard members to complete the deployment health assessments via the electronic deployment health assessment https://164.167.138.44/edha. The Coast Guard passphrase is Coastie1234(dollar sign). 6. Active duty Coast Guard members should report to their supporting HSWL clinic medical department for review of their completed deployment health assessment. (For active duty members enrolled in TRICARE Prime Remote, notification to the supporting HSWL clinic physical exam department of completion of assessment will result in telephonic review by the cognizant medical representative). Selected reserve members can call Logistics Health, Inc. at 1-888-PDHRA-99 to have their delinquent Deployment Health Assessments reviewed by contracted medical providers via telephone. 7.
