Coast Guard Medical Manual, COMDTINST M6000.1 (series) 1. This message updates the Coast Guard Medical Evaluation and Physical Examination Requirements (Ref (A)) for Reservists and Active Duty members who are changing status. 2. A completed Periodic Health Assessment (PHA) and Report of Medical Assessment, Form DD-2697, shall be used for the following: a. Reserve members who are being released from Active Duty Orders(greater than 30 days), b. Individual Ready Reserve (IRR) members who are changing status to Selected Reserve (SELRES) or Active Duty, c. Retired Reservists (RET-1 or RET-2) being recalled to Active Duty, d. Active Duty members changing status to the Ready Reserve (SELRES or IRR). 3. Separation or Retirement Examination Report of Medical History, Form DD-2807-1 and Report of Medical Examination, Form DD-2808: a. Shall be used for both Reserve members (on Active Duty Orders for greater than 30 days) and Active Duty members separating from the Coast Guard, retiring, or not planning to rejoin a military service. The separation and retirement examination consists of completion of the Report of Medical History, Form DD-2807-1 and Report of Medical Examination, Form DD-2808. Objections to assumptions of fitness at separation or retirement are addressed in Ref (B). b. May be used by Reserve members (on orders for 30 days or less) who are separating or retiring from the Ready Reserve (SELRES or IRR). Separation or retirement examinations are optional for Reserve members on orders for 30 days or less. 4. Members identified in paragraph 2 are authorized to call Logistics Health Incorporated (LHI) at 1-888-MYPHA99 (1-888-697-4299) to make an appointment to get their PHA and Report of Medical Assessment, Form DD-2697 completed. Members must: a. Inform LHI they are in one of the categories mentioned in paragraph 2, b. Ensure they schedule their Report of Medical Assessment, Form DD-2697 and PHA appointment in a timely manner, c. Not bring their medical record with them - LHI will use the Medical Readiness Reporting System and self-reported information to complete the Report of Medical Assessment, Form DD-2697 and PHA. 5. Reserve members being released from Active Duty Orders (greater than 30 days) should arrange sufficient time to complete their requirements (PHA, Report of Medical Assessment, Form DD-2697) before their Release from Active Duty date. If reserve members do not plan accordingly, funding may not be available to continue them on active duty until completion of their Report of Medical Assessment, Form DD-2697 and PHA (signed by a Medical Officer or a LHI contracted healthcare provider). 6. Medical Officers and LHI contracted healthcare providers will clearly annotate in Block 20 of the Report of Medical Assessment, Form DD-2697 whether the member meets retention standards in accordance with Ref (B). 7.
