SUBJ: 2012 HEALTH SERVICES OF THE YEAR AWARDS
A.  Medals and Awards Manual, COMDTINST M1650.25 (SERIES)
1.  This ALCOAST solicits applications for the Michael J. Cristy
Health Care Facility of the Year (HCFOTY) Award, Clinic Health
Services Technician (HS), and Independent Health Services Technician
(IDHS) of the Year (OTY) awards for 2012.  Units that support any of
the above entities are encouraged to review chapter 6.K.1 through
6.K.3 of REF A.
2.  Clinics should forward their HCFOTY and clinic HSOTY of the Year
nominations to COMDT (CG-1121).
3.  Nominations for IDHSOTY (afloat or ashore) should be routed from
the unit to COMDT (CG-1121).
4.  Selectees for HSOTY and IDHSOTY will be automatically nominated
for the Lewis Seaman Award for Outstanding Operational Support.  This
is an annual award for enlisted personnel presented by the
Association of Military Surgeons of the United States.
5.  Nomination packages must be received by COMDT (CG-1121) NLT 8
February 2013.
