SUBJ: UPDATE TO THE LATERAL CHANGE REQUEST FORM
1. The Lateral Change Request form is now an official Coast Guard
form, CG-1002. It is located in the electronic forms library at
http://www.uscg.mil/forms.
2. Effective immediately, discontinue use of all previous versions of
the form.
3. Additional information on the Lateral Change Request form is
available at http://www.uscg.mil/rpm/rpm1/rcc.asp.
