An approval memorandum and CG-9556 (Acceptance and Oath of Office) will be delivered by email to each officer. The authorized rank and date of rank will be indicated on both the memo and the CG-9556 and may differ from the rank indicated above. For officers still on active duty, commands must confirm by email to the POC in paragraph 8 that the officer: A. Complies with Ref (A) and weight is recorded in DA. B. Is not the subject of, or party to, an investigation that could result in adverse action against the officer. C. Is not pending proceedings under the UCMJ. 3. An officer who accepts the reserve appointment must: A. Follow the procedures outlined in the memo. B. Return one signed legible CG-9556 to CG PSC-rpm-1 by email attachment to arl-pf-cgpsc-rpm-boardspanels(at)uscg.mil for inclusion in the official CG PSC Personnel Data Record for the officer. C. Provide CG PSC-rpm-1 with an email attachment containing an approved (endorsement in block 25) report of Medical Assessment Form, (DD Form 2697) completed within the last 12 months, indicating the officer is qualified for retention and qualified for service in block 20. To affiliate with the Ready Reserve, officers must meet retention, not separation criteria, and be fit to mobilize. D. Contact the reserve assignment officer at arl-sg-cgpsc-rpm-2-jo (at)uscg.mil to determine the availability of drilling positions and if a Selected Reserve (SELRES) position is desired. 4. CG PSC-rpm-1 will complete the reserve component accession processing in DA only for officers who provide an executed CG-9556. 5. Officers who expect to be in zone for selection during promotion year 2014 should contact CG PSC-rpm-1 immediately. Accession into the reserve component must be completed prior to the announced selection board convening dates. 6. Unless otherwise indicated, regular officers who do not respond within 60 days from the receipt of the memo and CG-9556 will have their records closed.
