Sponsors Signature
All Sponsors must be present with their dependents, or have their dependent bring in a DD Form 1172 (Application for an ID Card) signed by the Sponsor before an ID card can be issued.
NOTE: The sponsor must sign the DD Form 1172 in the presence of the verifying officer who prepares the form in an ID card office by using DEERS. A DD Form 1172 that has not been signed, by the sponsor, in front of an ID Card Verifying Official ,and is presented to the ID card issuing office by someone other than the person authorized to sign block 90 must have a notarized signature. This would occur if the ID card office prepares the form and then mails it to the sponsor, the sponsor must then get their signature notarized. You The Sponsor Must sign the DD Form 1172 in the presence of the Verifying Officer, if not--- we cannot LEGALLY issue them the ID Card
The only exceptions are:
- Member is DECEASED
- Spouse relationship to member is "URW - UMW - URFSO - UMFS
This is in accordance with Department of Defense and Department of Homeland Security, which states:
Block 90, Signature
The sponsor's signature certifies acknowledgment of conditions and responsibilities under which benefits and privileges are authorized for eligible dependents. Therefore, the sponsor must sign block 90 with the following exceptions:
a. When the ID card application is for an unremarried or unmarried former spouse.
b. When the sponsor is deceased, the surviving spouse signs the DD Form 1172.
c. When the sponsor is deceased and there is no surviving spouse, children over 18 years old may sigh the form.
d. When there is no surviving spouse and the unmarried child is under 18 years old, the parent or guardian signs the form.
e. When the DD Foprm 1172 is prepared for the purpose of terminating eligibility and the verifying official has viewed the appropriate documentation, the verifying offical may sign.
f. When a valid power of attorney is presented
Social Security Numbers
Social Security Numbers are now required for all dependents. If you have a newborn and you have requested a SSN, you need to get that number to an ID office as soon as possible to have it placed in the Dependent's DEERS record. Dependent SSN's are now being used for further identification and are also now being placed on the Automated Dependent ID card.
RAPIDS Definitions
The following list of RAPIDS definitions is for informational purposes only.
Adopted Child. A child adopted before the age of 21, except a child with an incapacitating condition that existed before the age of 21, may be adopted after age 21.
Annulled. The status of an individual, whose marriage has been declared a nullity by a court of competent jurisdiction. That restores unremarried status to a widow, widower, or former spouse for reinstatement of benefits and privileges.
Attainment of Age 65. The first day of the month of the anniversary of the sixty-fifth birthday, unless the birthday falls on the first of the month. If the birthday is the first of the month, attainment of age 65 occurs on the first day of the preceding month.
Dependent. Individuals, whose relationship to the sponsor leads to entitlement to benefits.
Documentation. The following is some of the documentation which MAY BE required for the particular card the member or dependent is trying to obtain: Properly certified birth certificate or certificate of live birth authenticated by attending physician or other responsible person from a U.S. hospital or a military treatment facility showing the name of at least one parent; properly certified marriage certification; properly certified final decree of divorce, dissolution, or annulment of marriage and statements attesting to nonremarriage and status of employer-sponsored health care; court order for adoption or guardianship; statement of incapacity from a physician or personnel or medical headquarters of sponsor's parent Uniformed Service; letter from school register; retirement orders (providing entitlement to retired pay is established) or DD Form 214; DD Form 1300, "Report of Casualty" certification from the VA of 100-percent disabled status: orders awarding medal of Honor; formal determination of eligibility for Medicare Part A benefits from the social Security Administration; civilian personnel records; and invitational travel orders.
Dual Status. A person who is entitled to privileges from two sources (e.g., a retired member, who is also the dependent of an active duty member; a member of a Reserve component who is an eligible dependent of a military sponsor; or a child, who is the natural child of one sponsor and the stepchild and member of a household of another sponsor).
Former Spouses. Individuals who were married to a Uniformed Service member for at least 20 years, and the member had at least 20 years of service creditable to wards retirement, with at least a 15-year overlap of their marriage and the members military service.
Inactive National Guard (ING) . The ING is part of the Army National Guard. Those are Reserves, who are attached to a specific National Guard unit, but do not participate in training activities. On mobilization they shall mobilize with their assigned units. Those members muster with their units once a year. Issuance of DD Form 1173-1 to ING dependents is mandatory. The ING participates in the Guard or Reserve DEERS Enrollment Program.
Individual Ready Reserve (IRR) . Trained individuals, who have previously served in the active component or Selected Reserve and have time remaining on their Military Service Obligation (MSO). It also includes volunteers, who do not have time remaining on the MSO, but are under contractual agreement to be a member of the IRR. These individuals are mobilization assets and may be called to active duty. Issuance of DD Form 1173-1 to IRR dependents is mandatory. The IRR participates in the Guard and Reserve DEERS Enrollment Program.
Institution of Higher Learning. A college, university, or similar institution, including a technical or business school, offering post secondary-level academic instruction that leads to an associate or higher degree if the school is empowered by the appropriate State education authority under State law to grant an associate or higher degree. When there is no State law to authorize the granting of a degree, the school may be recognized as an institution of higher learning, if it is accredited for degree programs by a recognized accrediting agency. The term also shall include a hospital offering educational programs the post secondary level regardless of whether the hospital grants a post secondary degree. The term also shall include an educational institution that is not located in a State, that offers a course leading to a standard college degree, or the equivalent, and that is recognized as such by the Secretary of Education (or comparable official) of the country, or other jurisdiction, in which the institution is located. Enrollment must be full-time, which at the undergraduate level is 12 semester hours and at the postgraduate level is 9 semester hours.
Issuing Official. A person who is a U.S. military member, DOD civilian (appropriated or non appropriated fund supported) or equivalent civilian personnel employed by the National Guard of the United States and responsible for issuing DD Forms 2, DD Form 1173, and DD Form 1173-1 to bonifid beneficiaries, as determined by the verifying official, and the only person authorized to sign block number 108, on the DD Form 1172.
Member. An individual who is affiliated with a Service, either active duty, Reserve, active duty retired or Reserve retired.
Ready Reserve. Military members of the Reserve and National Guard, organized in units or as individuals, liable for recall to active duty to augment the active components in time of war or national emergency. The Ready Reserve consists of three Reserve component sub categories: the Selected Reserves (SelRes), the IRR, and the ING.
Reserve Retired Entitled to Pay at Age 60 (Gray Area Retirees). Those retirees, who have completed 20 qualifying years for retirement and are entitled to receive retired pay at age 60. those individuals, who are mobilization assets and may be recalled to active duty under 10 U.S.C. Chapter 39. Issuance of DD Form 1173-1 to dependents of Gray Area Retirees is mandatory. The group participates in the Guard and Reserve DEERS Enrollment Program.
Selected Reserve (SelRes). Those units and individuals within the Ready Reserve designated by their respective Services, the Chairman of the Joint Chiefs of Staff, and approved by the Chairman of the Joint Chiefs of Staff, as so essential to initial wartime missions that they have priority over all other Reserves. They must be prepared to mobilize within 24 hours. Issuing DD Form 1173-1 to their dependents and participation in the Guard and Reserve DEERS Enrollment Program are mandatory.
Standby Reserve. Personnel, who maintain their military affiliation without being in the Ready Reserve, who have been designated key civilian employees, or who have a temporary hardship or disability. Those individuals are not required to perform training and are not part of units. Those individuals are trained and could be mobilized, if necessary, to fill manpower need in specific skills. Those individuals do not participate in the Guard and Reserve DEERS Enrollment Program.
Unmarried. A widow or widower, who remarried and that marriage terminated by death or divorce, or a former spouse, whose subsequent remarriage ended by death or divorce.
Unremarried. (a) A widow or widower who has never remarried. (b) A former spouse whose only remarriage was to the same military sponsor is treated as if he or she never remarried and the periods of marriage may be combined to document eligibility for former spouse benefits.
Verifying Official. A person, who is a U.S. military member, DOD civilian (appropriated or non appropriated fund supported) or equivalent civilian personnel employed by the National Guard of the United States or other similarly qualified personnel in exceptional cases as determined by the Secretary of the Military Department, or a designee, and responsible for validating eligibility of bonifid beneficiaries to receive benefits and entitlements, and the only person authorized to sign block number 99 on DD Form 1172.
Ward. An unmarried child whose care and physical custody has been entrusted to the sponsor by a legal decree or other instrument that a court of law or placement agency issues. This includes foster and pre adoptive children and children for whom a managing conservator has been designated. Wards must be dependent on the sponsor for over half of their support.
