Conditions for Issuance of Dependent ID Cards

  1. Any change in the sponsor's status affecting the use of the card should be reported to the nearest Identification Card issuing facility.

  2. It is unlawful for a person to use a card to which they are not entitled.

  3. No person shall have in their custody more than one valid DD Form 1173 in their own name.

  4. The loss or theft of an identification card will be reported promptly by the individual concerned through the resubmission of a DD Form 1172 at the nearest ID Card issuing facility.

Documentation Needed for Issuance of an ID Card

The DEERS check is the first source checked for verification, and if the dependent is enrolled, it is the only substantiation required.

Re-issue of Dependent ID Card
Dependent ID Cards (DD Forms 1173 & 1173-1) ARE NOT valid documentation to issued ID cards.

Spouse
Marriage Certificate or state certified/sealed Marriage License.

Notes:
1) The DD Form 1173-1 cannot be issued to an Unremarried Former Spouse (URFSO) nor to a Parent or Parent-in-Law.

2) Widow of a Deceased RESRET Member: A RESRET sponsor who dies while in the RESRET status remains a RESRET on DEERS until the anniversary of their 60th birthday. The RESRET dependents are issued a DD Form 1173-1 which entitles them to Commissary, Theater and Exchange privileges prior to the 60th birthday. Upon the anniversary of the deceased RESRET sponsor 60th birthday, the Unremarried surviving spouse, children, step-children and ward will receive their initial issue of a 1173 (Dependent card) which authorizes medical.

3) Lawful Spouse, Including Common Law Spouse: A spouse through either legal or common law marriage. To be valid, common law marriages must have been entered into a state that recognizes common law marriages.
